PHD Medical is committed to ensuring the protection of personal health information for the provision of health services.

Etrending complies with the data confidentiality, integrity, security and privacy standards using the guidelines of the Health Portability & Accountability Act (HIPAA).

This Privacy Policy applies only to personal information collected from individual customers. It does not apply to corporate or business contact information. In addition, this policy is not applicable to personal information that has been aggregated or made anonymous and no longer specifically relates to an identifiable individual. Etrending reserves the right to use aggregated or anonymous information in any manner it considers appropriate.

When and Why Etrending asks for specific types of personal information
In a few areas on our Web site and online customer support tools, as well as in our telephone interaction with customers, we ask you to provide information for a number of purposes including in order to enable us to proceed with processing and fulfilling your product or service purchase, to assist you with technical support issues, or to follow up with you after your product or service inquiry or purchase to obtain your feedback or to ensure that you are aware of Etrending's product and service portfolio and any special offers or promotions we think may be of interest to you. In certain circumstances, it is completely optional for you to provide your personal information, and in other circumstances, such as in order to complete your purchase, such information must be provided. Under no circumstances do we sell our customer's personal information to third parties.

Disclosure to Service Providers/Partners
Etrending will share your personal information with certain select service providers/partners in order to support your customer relationship with Etrending . For example, service providers will be engaged to confirm your identity and address, to assist in the order fulfillment process, repair products purchased, to perform credit analyses, and to assist in alerting you to product and service offerings that we think may be of interest to you. All such service providers only use your information for the purposes and in the matter we have instructed them. They have also committed to keeping your personal information confidential, using it only as directed by Etrending and to secure your personal information in a manner that is consistent with this Policy.

Your Right of Access to Your Personal Health Information
Etrending will permit you the reasonable right of access and review of your Personal Health Information. We will endeavor to provide the information in question no later than 30 days following receipt of your request. If we require additional time, or if we feel that we have grounds to refuse your request, we will notify you of new timing for our reply and/or the reasons for our extension or refusal. Etrending reserves the right to charge for copies of your records, however we will always let you know in advance if there is any such charge. Etrending will provide information from its records in a form that is easy to understand. To protect you, Etrending may require sufficient information to allow us to confirm that the person making the request is authorized to do so before granting access or making corrections.

For How Long and Where Does Etrending Keep My Personal Information?
We keep personal information only so long as is needed in order to meet the purposes set out in this Policy, including to enable us to meet any legal or regulatory requirements. All data submitted to Etrending will be kept for a minimum period of seven years unless Etrending agrees to a request to remove said data.

The principal places in which Etrending holds personal information are in Montreal, Quebec and nearby municipalities where off-site storage facilities may be located. Because Etrending is part of a global organization, personal information may be shared (including for storage and processing) among Etrending affiliated companies in other countries including, but not limited to, Canada and the United States. Etrending entered into agreements with such companies and has taken the measures described below (See " Etrending and Related Companies") in order protect your personal information.

You can request to have factual inaccuracies in this information corrected by contacting privacy@etrending.com As well, for both such requests, you can make such request by writing to us at the address provided at the end of this Policy to the attention of Privacy Compliance Representative.

Etrending and its Related Companies
Etrending is a global organization and as such, a number of our business processes relating to our products and services occur on a global level. Etrending is a wholly owned subsidiary of PHD Medical Inc., based out of Montreal, Quebec. In addition, there are a number of Etrending affiliated companies throughout the world. Your information will be shared with PHD Medical Inc. and such affiliated companies in order to assist us in providing the products and/or services that you have requested. Such companies will only use your information for the purposes we have agreed upon and for no other purposes. As a number of these companies are situated outside of Canada, we have agreements in place requiring them to use the information only for the purpose for which it was provided and to have appropriate safeguards for the protection of that Personal Information.
